All prospective suppliers should sign, stamped and submit the duly filled Supplier Registration Application. (Page 1-5)

All prospective suppliers should submit the requested documents (if applicable) mentioned in the check list with the duly filled registration form. (Page 5)

• Letter Confirming Bank Details (mandatory)
• Certified Copy of Business Registration/Certificate of Incorporation (mandatory)
• Certified True copy of Form 20 (Latest) / Copy of NIC for Partnerships/Proprietorship (mandatory)
• Copy of NBT Registration
• Copy of VAT Registration
• Certified Copies of your company’s last three year's balance sheet and income statement
• Organization Chart
• List of Major Customer
• Mobitel Categories Form
• Certified True copy of Form 15

If any prospective supplier is unable to submit all requested documents, please provide reasons as to why the documents will not be submitted.

The last page of the application should be signed with a dull signature by the authorized signatory and the company stamp should also be placed.

All prospective suppliers should clearly indicate the Main category & the subcategory of the business area that the supplier expects to engage with Mobitel in the category form. (Page 1-3)

• All prospective suppliers should hand over Two (02) copies of duly signed NDA.
• Failing to submit any of the documents requested by Mobitel may lead to the rejection of the request for registration.
• For certain categories, Mobitel may carry out a physical inspection/sample before the supplier is being registered.
• On successful registration, the prospective supplier will be notified by email notification.
